From 1st April 2011, local libraries (instead of the borough or district councils) will be issuing bus passes for older people or those with disabilities.

 

People who already have a bus pass can use their passes until the date runs out. 


There are three easy ways to get your free bus pass:-

 

* Find out what you need to get your free bus pass on-line or by phone

* Check what papers you need to take with you

* Visit your local library to get your bus pass

The forms below follow a travel training process from the first contact with someone to completion of their training for a single journey.

 Forms which travel learners may complete have been done in two formats:

  • with pictures
  • mainly text

Form 1 Initial Referral Form is for Surrey County Council day service use only to be raised as an issue in reviews.

ALL TO TAKE CAREFUL NOTE: Surrey County Council strongly advises each person (trainer) proposing to use these resources to take steps to ensure he or she has a thorough understanding and knowledge of the individual student and their needs and abilities.  Further the trainer applies their own training skills, abilities and judgement when training and their own judgement in respect of the traffic conditions and the suitability of the environment for training to take place.

Surrey Travel Network Forms and all associated guidance on the website is intended to suggest guidance and best practice and to provide tools to support travel training activities.  Where indicated on the forms, they have been based on the Bradford Travel Training Unit format, which has been used with kind permission of Bradford Travel Training Unit.
